RECREATIONAL TEAM SPONSORSHIP FORM

Each year many local businesses take advantage of having
10-20 soccer players running around advertising their company wearing printed shirts.
Of course this is not the only reason to sponsor a team....
when you sponsor a team you are helping the community in which
you live or work and the Canfield Soccer Club appreciates all of you!
At the end of every season, we are pleased to present an appreciation plaque for
you to proudly display...and as you can see we will list you here on our website
encouraging everyone to patronize your business.

Note: Sponsorships are on a first come first serve basis. So if you would
like to sponsor your child's team or choose a particular color shirt please
submit your form with payment as soon as possible.

Please print and complete theTeam Sponsorship Form and mail with your check to
the address on the form.
