IF YOU ARE A COACH OR ASSISTANT COACH:
You must complete a paper registration form and an online Risk Management Statement.
The paper form is for the Soccer Club's file and the online form is at a secure website and is
strictly for the safety of our children.
YOU CAN NOT COACH IF THESE ARE NOT COMPLETED ON OR BEFORE MARCH 31, 2010.

NOTE: THIS YEAR THERE IS A CHARGE OF $5 TO THE CANFIELD SOCCER CLUB TO
CONDUCT A BACKGROUND CHECK FOR ALL COACHES. THE CLUB IS PAYING FOR THIS
CHARGE THIS YEAR, BUT YOU WILL NEED TO ENTER CODE #05379 WHEN PROMPTED TO
IN ORDER FINISH WITHOUT BEING CHARGED.

As a coach you are responsible for contacting your players in a timely manner once you receive the
final players list. The players are anxious to hear from you so please contact them and depending
on age, speak with the parents as well. Some coaches set-up e-mail databases for communication,
others have assistant coaches or parents help with phone calls to advise practice times and
locations etc. Practice is important to a team's overall health and excitement. Plan on have at least
4-5 team practices before the first game. This is another reason for early contact. Set dates
in advance as the summer is a busy time for families. For example..we will practice every
Thursday at 6:00 Fair Park. You may practice wherever you can find a field: The Fairgrounds,
High School, Fair Park, this is up to you.
YOU OR A DESIGNATE MUST PICK UP TEAM UNIFORMS
BE SURE YOUR TEAM IS AWARE OF PICTURE DAY.
